Family Choice Home Care was founded in 2008 as a locally owned personal care agency, built to serve families throughout the Bluegrass region and beyond. We are not a franchise, which means every client receives individual attention rather than a standardized service plan handed down from a corporate office.

Our agency is still owned and operated by Sybil Forsythe, MS CCC/SLP, whose clinical background spans home health, skilled nursing and rehab, hospital and acute care, and long term care settings, working directly with elderly and disabled individuals throughout her career, including specialized experience in Alzheimer’s care, geriatrics, and neurological conditions.

That clinical foundation shapes how we approach care today. Beyond personal care, we also provide outpatient and contract rehabilitation therapy, including speech language pathology, physical therapy, and occupational therapy, giving us a deeper understanding of the challenges that follow a stroke, hospitalization, joint replacement, or Parkinson’s disease. That insight helps us build more personalized care plans that support real recovery, so families can keep their loved ones safely and comfortably at home.

We also welcome healthcare facilities in need of short term or long term therapy support to reach out and learn how our contract therapy team can help, an area of care many home care agencies simply cannot offer.
